Narrative

An employee was on top of a temporary HVAC/air handler unit, trying to move hoses with help from two other employees. The injured employee was pulling on a zip tie around the hoses when the zip tie broke. The employee jumped off the unit and rolled his ankle as he landed, suffering ankle/foot fractures that required surgery.
