Narrative

On August 31, 2024, at approximately 12:10 AM, an employee was correcting an alignment issue with a stack of pallets that were loaded in the pallet dispenser. His left index fingertip was pinched between the pallet support plates (flapper) and the flapper stop. The employee sustained a fingertip amputation.
