Narrative

An employee was guiding a concrete lid that was suspended from the boom of an excavator by chain straps and pins. A pin failed, causing the lid to fall approximately 1 foot and land on the employee's left foot. The employee was hospitalized with a broken toe and a crush injury to the upper left portion of the foot.
