Narrative

On October 9, 2024, at approximately 2:00 AM, two employees were using a forklift to pull a dead forklift to a maintenance area. A pin from the strap securing the dead forklift broke off and struck the forklift operator's left tibia, resulting in a fracture. The employee was hospitalized.
