Narrative

An employee was helping a forklift operator load concrete blocks onto a flatbed trailer for delivery. The employee was placing blocks at the bottom of the cube when a 25-pound block came free about 2 feet above the blocks he was placing in the cube and the block struck his left thumb. The employee sustained an amputation of the thumb tip and the loss of the fingernail.
