Narrative

An employee was working from an aerial lift removing tree limbs, approximately 60 feet from ground level. The front end of the lift truck's chassis began to slide sideways on ice and snow. The movement caused the outrigger to slide off the outrigger pad, which in turn caused the truck to tip over onto its side. The top of the aerial lift basket struck the employee's mouth, and the employee suffered broken front teeth, closed facial fractures, and a fracture of their lateral right orbital wall.
