Narrative

An employee was working with a piece of metal bar stock being fed into a rotary straightener. The employee was adjusting the bar stock in the feeder. The straightener caught, twisted, and pulled the bar stock, which struck the employee's hand. The employee suffered a partial amputation to the thumb.
