Narrative

An employee was moving a step ladder to a different location. A brad nail gun with an air hose attached was hanging on the side of the ladder. When the employee climbed the ladder, his right leg hit the safety tip of the nail gun and the nail gun shot a brad nail into the employee's right leg.
