Narrative

An employee set up an extension ladder and climbed it to access a work area/platform. After reaching the top of the ladder, the feet of the ladder slipped and the employee fell approximately 15 feet to the ground. The employee sustained a back injury, internal bleeding, and a collapsed lung.
