Narrative

On February 3, 2025, at about 5:00 PM, an employee was conducting maintenance on an overhead waste system when part of an air tube weighing about 300 lbs. fell about 3 feet onto the employee's right forearm. The employee was hospitalized with multiple fractures to the forearm.
