Narrative

A forklift was being used with a counterweight to punch out the wedge of a die. The forklift operator was working to adjust the counterweight when the forklift attachment bounced back. An employee's fingers were pinched between the forklift forks and the handle of the counterweight. The injured employee's left ring and middle fingertips were amputated.
