Narrative

An employee was using a 6,000-pound forging hammer to make flanges. They placed the metal on top of a flange die and started to hit the red-hot metal with the ram. During the hammering action, a shard of hot metal broke off the part being worked and punctured the employee's abdomen. The employee required surgery.
