An employee was loading a delivery truck. There were two 4x4 pieces of wood stacked on top of one another in the truck. The employee stepped on the wood, lost his balance, and fell from standing height onto a pallet jack inside the truck. His back hit the pallet jack and he sustained fractured ribs. The employee was hospitalized.
