Narrative

An employee was climbing up a ladder to service the top of a truck. He stepped from the ladder to a fuel tank when the ladder slipped and the employee fell 5 to 6 feet, hitting his back and side on the concrete floor. The employee suffered a fractured L2 vertebra.
