Narrative

An employee was loading a spherical plate onto a vertical lathe with an overhead crane. Hooks on the slings were attached to the plate. One hook was stuck. The employee was working to get the hook off the plate when the plate fell and pinched a finger on their left hand against the machine, resulting in a fracture/amputation.
