On 7/26/2018, an employee's finger was caught in an auger, resulting in a right middle finger amputation at the first digit. On 7/28/2018, an employee's fingers were caught in a belt and pulley powered by an electric motor, resulting in an amputation of the left index and middle fingers at the second knuckle. This record contains two separate events that were reported simultaneously to OSHA by the employer. These events are captured as one report.
